The witness can be any person over the age of 18. The witness must be physically present for the signing of the Deed Poll.

The service of a solicitor/Commissioner for Oaths is needed for the Affidavit of Attesting Witness.

The Change of Name by Deed Poll document must be completed in the presence of the selected witness.

The witness must swear the affidavit before the selected solicitor/Commissioner for Oaths.

A solicitor/Commissioner for Oaths or a member of An Garda Siochana must certify copies of the applicants birth certificate and photographic identification.

The applicant must complete the Request to Enrol form. This does not need to be witnessed and must be signed by the applicant.
