These forms must be completed and lodged with the Examiner’s Office when applying for a bankruptcy hearing date.
When filing documents with the Examiner’s Office, you must provide:
One original, and
Two copies of each document

The Bankruptcy Register Online contains details of personal bankruptcies in Ireland and is maintained by the Office of the Examiner.
